By filling out the form below, you give permission for a Benefits Coordinator from the Greater Chicago Food Depository to contact you via phone.

Our coordinators will conduct a pre-screening and inform you of benefits you are potentially eligible for and help you apply for benefits. The Illinois Department of Human Services will process the application and determine eligibility.

The Food Depository respects your privacy. We only share the information you give us with the Illinois Department of Human Services to help process your benefits. We never share information with any immigration or law enforcement agencies.
